Important Notes When Filling Out the Application Form:

• The application form must be filled out on a computer and in capital letters.
• A wet signature at the bottom of the form is mandatory.
• Turkish characters (ç, ğ, ı, ö, ş, ü, Ç, Ğ, İ, Ö, Ş, Ü) must not be used under any circumstances.
• Write TURKEY instead of Turkey.
• Write GREECE instead of Greece.
• In the citizenship section, write TURKISH (do not write TC, TR, Türkiye, Türk, etc.).
• Profession and/or job title must be written in English.
• Housewives must write house wife in English.
• The right sections on pages 1 and 2 of the form should not be filled out.
• Article 21: The travel purpose section must be marked as TOURISTIC; other options will not be accepted.
• Article 24: The requested number of entries must be marked as SINGLE ENTRY.
• Article 25: If accommodation is planned, the travel duration must include both arrival and departure days. (E.g., for June 1–5 write “5”, for same-day trips write “1” or “Same Day Return”.)
• Article 26: Dates of Schengen visas obtained in the last 3 years must be written and photocopies of these visas must be attached.
• Article 28: Leave blank.
• Article 31: Write the name of the hotel; the hotel’s address and phone number should be written on the next page. Important note: The country name on the hotel voucher must be in English; YUNANİSTAN is not accepted, it must say GREECE.
• Article 32: Only guests applying with an invitation fill this out; guests filling out Article 31 leave Article 32 blank.
• If the boxes in the form cannot be filled electronically, tick them with a black pen after printing.
• Article 37: Only signed by parents for children. Children do not sign. Adult applicants must sign the last page at the end of the form.
